Instructions for submitting your images:

Please complete our Interpretation Request Form. Print and submit with your actual imaging media.
All images must be DICOM compatible and presented on CD. Please note CDs will not be returned, so please copy before sending.
Mail completed forms along with CD to address noted atop form. It is recommended that you mail using a trackable delivery service such as UPS, FedEx, or Priority Mail via the USPS.
Payment is due at the time of service. We do not bill insurance or third parties.
In keeping with HIPAA regulations, a fax number must be provided for report delivery. Alternatively, online access can be provided upon request.
Corporate accounts are available for clinics with imaging systems that want to send on a regular basis via HIPAA-secure VPN. If you are interested in a corporate account, please complete this survey or call 866.674.9729.
